Alberta Government request for information

Alberta Occupational Health and Safety (OHS) is seeking information pertaining to a serious incident investigation with respect to the following details:

On September 12, 2017 between 8:00 p.m. to 9:30 p.m., a worker was fatally injured on the northbound section of Highway 2 approximately five kilometres north of Airdrie, Alberta.

OHS is attempting to locate a vehicle(s), possibly semi-truck with damage to the passenger-side of the vehicle (e.g., semi-trailer’s cab, exhaust stack, trailer, or etc.).

If an operator, contractor, and/or employer has sustained damage as per the above or has information of such damage, please contact John Kwan, OHS Officer, at john.c.kwan@gov.ab.ca  no later than July 31, 2018.

OHS will obtain further/additional information from above party(s) under the authority of Section 19, OHS Act (Revised Statutes of Alberta, RSA 2000 cO-2).
